Use this form to guide you through your first few days of employment at AASU

NOTE: This orientation is for full time employees who are benfits-eligible.
You will need to turn in EVERY PAGE INCLUDED IN THIS CHECKLIST. Incomplete packets will not be accepted!

_____ Complete online orientation including Right to Know Training, review of of policies, and benefits overview.
BE AWARE OF BENEFITS ENROLLMENT DEADLINES!

_____ Print and complete the employment paperwork:

_____ I-9 Employment Eligibility Verification

_____ Board of Regents Security Questionnaire & Loyalty Oath (this can be notarized by Human Resources)

_____ Form G-4 Employee Federal Witholding Allowance Certificate

_____ Form W-4 Employee Federal Witholding Allownance Certificate

_____ Direct Deposit Form

_____ Emergency Contact Form

_____ Dependant / Beneficiary Form

_____ Personal Data Sheet (2)

_____ Employment Verification Form (to be completed by AASU Department employee or Human Resources staff member)

_____ If you are benefits eligible, you should have received a benefits packet with booklets and forms, along with your official offer letter.
In your offer letter, a time is noted for your payroll / benfit session at Human Resources. Please have all of your employment forms completed
and bring them to this session; you may ask questions and complete forms to enroll in the benefits programs you decide to participate in.

_____ Meet with your supervisor to learn more about you department's mission and how you play a role in achieving that mission.

_____ Visit Public Safety to obtain your permit to park on campus. You'll need the Employment Verification Form
(completed by an AASU Department employee or Human Resources staff member)

_____ To obtain an email account, contact Computer & Information Services (CIS).

_____ Meet with the appropriate departmental representative for keys, building information, and other administrative policies that relate
specifically to your job.
